Registration and Enrollment for School Year 2020-2021
Kha'p'o Community School will begin registering and enrolling students for the upcoming school year. We welcome back all our students and look forward to meeting new students this fall!

Our registration process for students can be done through one of these pathways to register and enroll for fall 2020.
1. Download the forms available through the section that applies to your student below, fill out, and return to the school by mailing to our address:

2. If you are unable to download and print the forms, please contact 505-753-4406 and request a registration packet be mailed to you. You will need to provide a mailing address, specify what type of registration packet (NEW or RETRUNING student), and how many you need.

Transferring to a new school this year? If your student is not returning to KCS for school year 2020-2021, we are sad to see you leave but we would like to help you transition to your new school. A student withdrawal form is required to be submitted so we can assist you with transferring.

NEW STUDENTS:
All NEW students must complete all the forms 1-7 and provide the following documents 8-15 to the front office (click on the link in blue to download form):
- New Registration Form
- Media Release Form
- Title VI ED 506 Indian Student Eligibility Certification Form
- Tewa Name Memo
- Emergency Contact Form
- School Health Form
- Home Language Survey
- Proof of Physical Exam (documentation from physician)
- Proof of Food Allergy (documentation from physician)
- Copy of Current Immunization Records (documentation from physician)
- Copy of Certificate of Indian Blood (CIB) or Tribal ID (We will need a biological parent's CIB or Tribal ID, if the child is not enrolled.)
- Copy of Birth Certificate
- Copy of Individualized Education Plan, if applicable (transfer students from other schools)
- Copy of child’s records from the school most recently attended, if previously enrolled in a school. (k-6th grade)
- Copy of Guardianship/Custody Papers if applicable
RETURNING STUDENTS:
All RETURNING students must complete all the forms 1-4 and provide the following documents 5-10 to the front office (click on the link in blue to download form):
- Returning Registration Form
- Media Release Form
- Emergency Contact Form
- School Health Form
- Updated Proof of Physical Exam (documentation from physician)
- Updated Proof of Food Allergy (documentation from physician)
- Copy of Current Immunization Records (documentation from physician)
- Updated Title VI ED 506 Indian Student Eligibility Certification Form if Tribal enrollment status has changed for student or biological parent
- Updated Certificate of Indian Blood (CIB) or Tribal ID (Tribal ID update required for all Santa Clara Pueblo members per tribal memo to update CIB Number 2/29/2020)
- Copy of Guardianship/Custody Papers if applicable
